History

Botulinum toxin (Botox) is the star of cosmetic medicine products. It is the first step to take if you want the most natural aging treatment possible.

Its incredible properties make it a widely used product around the world because it presents few risks, and with natural and harmonious results, when performed by a specialist plastic surgeon.

Many indications have emerged since the discovery of this product, including in aesthetic medicine (aging, eye opening, corner of the mouth treatment, sweat treatment….).

What is botox

It is a common misconception that Botox “freezes” the face and prevents natural facial expressions.

Botulinum toxin, or botulinum toxin, is a substance that acts on the neuron-muscle junction to obtain a muscle relaxant effect and thus temporarily reduce muscle contraction. At the doses used in aesthetic medicine, it blocks the contraction of the muscle at rest but not in action, for a more natural effect.

This neurotoxin is produced by an anaerobic bacterium, the Clostridium botulinum and is therefore a natural toxin.

Botox is a neuromodulator. Injected into specific points on the face, it interrupts the nerve signals that tell muscles to contract. Properly administered, it simply allows the muscles to relax for a smoother foundation. This is the anti-wrinkle injection!

Because each rejuvenation plan is customized, the length of the Botox treatment varies, but an injection session typically takes an hour or less. There is virtually no downtime – you can return to work or other activities immediately and apply makeup if you wish. You begin to see changes within two days and the improvement continues for a month. Most patients are so pleased with the results that they have scheduled touch-up sessions every three to four months or so.

Which indication for Botox?

Treatment of forehead wrinkles

The contraction of muscles in the upper third of the face, induced by repetitive facial expressions, is the cause of expression lines. The best indications for Botox injections are horizontal wrinkles that cross the forehead and frown lines situated between the two eyes (vertical interbrow wrinkles).

To avoid this undesirable effect, Dr. Gianfermi developed the micro- or derma-botox technique, which consists of injecting into the superficial layers of the skin – not the muscle – to respect the natural mobility of the face.

Dr. Gianfermi’s goal is to calm the hyperactivity of the forehead muscle to erase wrinkles but without freezing the facial expression.

After a botox injection session, the wrinkles gradually fade, with the result being optimal on the 8th day. The skin appears refined, smoothed, rejuvenated!

Beware, The effect of the product begins to fade after the 5th month: another injection is then be performed to maintain the effectiveness of the anti-wrinkle treatment.

Note : “sleep wrinkles” are caused by bad skin positioning at night. Over the years, vertical folds become imprinted on the skin and develop into deep wrinkles.

In this case, hyaluronic acid injections filling in skin breaks are more appropriate than Botox.

Treatment of the crow's feet wrinkle

Crows feet wrinkles are caused by the action of the orbicularis muscle, which, when contracted, forms striae at the corner of the eye. Repeated facial expressions, laughter and smiles throughout the day create these wrinkles that crease over the years with the loss of the skin’s natural elasticity.

The botulinum toxin injections are very effective in blurring them provided they are subtly performed.

The challenge is to not freeze the smile and to avoid disharmony with untreated lower eyelids that will then appear even more wrinkled.

After the injection session in the crow’s feet wrinkles, the skin break gradually disappears, the result being optimal by the 7th dayr. The eyes look younger!

The effect of the product begins to fade afterthe 6th month: another injection can then be performed to maintain the effectiveness of the anti-wrinkle treatment.

Botox injection should be done with caution in this area, ideally by an ocular plastic surgeon, as it can cause or worsen an existing malar pocket. So be careful!

Glabellar lines (or frown lines)

Frown lines, or glabellar wrinkles, are caused by contraction of the procerus muscles or corrugator muscles. They can be treated with botulinum toxin and/or hyaluronic acid injections.

Depending on the case, Dr. Gianfermi will choose one or the other method or combine both.

Perioral wrinkles

With age, wrinkles appear around the mouth giving a “puckered” appearance related to dehydration and thinning of the lips.
Their appearance is favored by contractions of the orbicularis muscle of the lips, especially in smokers due to repeated mouth movements (smoker’s wrinkles).

Injection of botulinum toxin calms the hyperactivity of the orbicularis muscle of the mouth and relaxes the peribuccal area. The practitioner must be careful not to inject the lip directly so as not to paralyze it.

After an injection session in the perioral wrinkles, the skin breaks disappear, the area around the mouth is magnified and rejuvenated!

The effect of the product begins to fade after the 6th month: another injection can then be performed to maintain the effectiveness of the anti-wrinkle treatment.

Note : Botox can also be used to correct a gummy smile that shows too much of the gums when you smile. The product injected into the lip frenulum prevents the upper lip from lifting up more than necessary.

The smile is thus corrected and beautified in a discreet and natural way.

Chin wrinkles

By contracting, the chin muscle can generate irregularities, small holes in the chin, an “orange peel” appearance that is corrected with botulinum toxin injections.

Botox Injection

Before a Botox Injection
- An initial appointment will be made by Dr. Gianfermi to target your requests exactly and to detect possible contraindications (pregnancy, known hypersensitivity, herpes, neuro-muscular disease, too recent injections...)

- Before/after photos of the botulinum toxin injection will be taken. There is no specific preparation to be done before an injection, nor do you need to be fasting.

- The injections are not very painful so very rarely require local anesthesia, which in this case will be performed by a cream applied 30 min before or after an injection of localized anesthetic
Botulinum toxin injection
The injections are performed in Dr. Gianfermi's office. The injection method is Dr. Gianfermi's own, which he has adapted to obtain the best results. The number of syringes will obviously depend on the number of areas to be treated. The area is anesthetized if necessary. The injection is done with a very fine needle so that it is painless.

The duration of a botulinum toxin injection session can vary from 10 minutes to one hour depending on the number of areas to be treated and the associated procedures such as the injection of hyaluronic acid.

The patient can apply makeup immediately after the session.
After an injection of Botulinum toxin
The aftermath is very simple and painless.
Just after the injection, an edema, a redness, or more rarely a slight ecchymosis can appear and which disappear quickly.

Make-up can be put back on immediately. Sports and lying down should be avoided within 2 hours of the injection.

In the first 24 hours, avoid wearing helmets or tight hats.

Avoid any risk of bruising or bleeding in the next 15 days (aspirin, anti-inflammatories, contact sports...).

Other Indications

Oily skin, large pores

Superficial injections of Botox may be indicated in people who have very oily, seborrheic skin on which enlarged pores are visible. Botox decreases sebum production and prevents acne.

Botulinum toxin is diluted in saline and then injected into the surface layer of the skin using the classic method
